Research Project Consent Form
The purpose of this research project is to gather information on the strengths and needs of college and university professors when delivering online learning, as well as to help validate the elements of the P.I.E. Model. Your responses will also identify areas for improvement and future research. This is an independent research project being conducted by Hugh Kellam, a part-time professor at the University of Ottawa.
You are invited to participate in this research project because you are a college or university professor who is currently teaching or has previously delivered a course via online learning.
Your participation will involve filling out two research surveys. The first begins on the next page, and asks questions regarding your online learning experiences based on elements of the P.I.E. Model. The survey should only take 10-15 minutes to complete. After completing this first survey, you will be provided with a complementary copy of the P.I.E. Model book. You will also be provided with a survey link to complete after you have read the book. Your responses in both surveys will be compared and contrasted and are important for this research study.
Your participation in this research study is voluntary. You may choose not to participate. If you decide to participate in this research survey, you may withdraw at any time. If you decide not to participate in this study or if you withdraw from participating at any time, you will not be penalized.
All survey responses will be confidential and we do not collect identifying information such as your name, email address or IP address.
We will do our best to keep your information confidential. All data is stored in a password protected electronic format. To help protect your confidentiality, the surveys will not contain information that will personally identify you. The results of this study will be used for scholarly purposes only.
